Consent and Release of Liability (18 & Over)

TO WHOM IT MAY CONCERN: The undersigned (“Participant”), plans to attend and participate in an event sponsored by THE CHRISTIAN YOUTH, GENERAL COUNCIL CHURCHES OF GOD, (Seventh Day), Inc. (Christian Youth).

LIABILITY RELEASE: In consideration of the Christian Youth allowing the Participant to participate in youth ministry events, I, the undersigned, do hereby release, forever discharge and agree to hold harmless Christian Youth, its directors, employees, volunteers and agents (collectively herein the “Christian Youth”) from any and all liability, claims or demands for accidental personal injury, sickness or death, as well as property damage and expenses, of any nature whatsoever which may be incurred by the undersigned Participant while involved in the youth ministry events. I, the Participant, plan to participate fully in youth ministry events, including trips away from the Council premises. Furthermore, I, the Participant, hereby assume all risk of accidental personal injury, sickness, death, damage and expense as a result of participation in recreation and work activities involved with youth ministry events therein. The undersigned further hereby agree to hold harmless and indemnify said Christian Youth for any liability sustained by said Christian Youth as the result of the negligent, willful or intentional acts of said Participant, including expenses incurred attendant thereto.

EARLY RETURN HOME POLICY: Should it be necessary for the undersigned participant to return home early due to medical reasons, disciplinary action or otherwise, the undersigned shall assume all transportation costs and responsibility.

TRANSPORTATION PERMISSION: The undersigned does agree to ride in any vehicle driven by an approved ADULT chaperone while attending and participating in youth ministry events sponsored by Christian Youth. I understand that SEAT BELTS SHALL BE WORN AT ALL TIMES during transportation and am responsible for compliance.

PHOTO RELEASE: The undersigned additionally gives permission for the Christian Youth to publish photographs or video of the undersigned participant on the Christian Youth website, social media and other Christian Youth related media (i.e. ACTS magazine youth reports).
